| 1.申請者可於2023年10月26-27日早上9時至下午5時及2023年10月28日上午9時下午12時 |
| 期間遞交申請表格（網上填寫或親臨本校）。 |
| Applicants are advised to submit the applications from 2023-10-26-27, {Weekday} (09:00-17:00) to 2023-10-28, {weekday} (09:00-12:00). |
| 2.報名費用港幣40元正（不獲退回及不可轉讓） |
| Application Fee: $40 (Non-refundable and Non-transferable) |
| 3.網上報名過程中家長只須上載學生証件相 |
| Only Personal Photo to be upload during online application. |
| 4.遞交方法：親臨本園遞交申請表格及攜同以下文件： |
| Return method: Submit the application form together with the documents required (including identity document) in person |
| 4.2.1 出世紙/居港證件正本及副本 |
| Original and a copy of the Child’s Birth Certificate. Children who were not born in Hong Kong will also need a copy of the child's passport pages showing the Child's photograph ID and Hong Kong Visa approval stamps (showing the authorised residency status) |
| 4.2.2 天主教領洗紙證書正本及副本(如適用者) |
| Original and a copy of the baptism certificate (For Catholic child only) |
| 4.2.3 兒童健康院打針紀錄証明正本及副本 |
| Original and a copy of the Child’s Vaccination Record |
| 4.2.4 曾就讀或現在就讀本園之兄弟姊妹或父母之證明文件正本及副本。如：畢業證書或其他証明文件。 |
| Original and a copy of Documentation if a parent or their child has graduated from this school |
| 4.2.5 居住地址證明文件正本及副本(最近3個月) |
| Original and a copy of Proof of Residential Address document(within 3 months) |
| 4.2.6 回郵信封1個(請用長信封寫上回郵地址及貼上$2.2郵票) |
| 1 Self-addressed envelope (size 9” long x 4” wide, with $2.2 stamp applied) |
| 4.2.7 證件相片2張(2吋 x 1.5吋) |
| 2 photo of Applicant (size in 2” X 1.5”) |
| 5.報名後如有任何資料需要更正或後補（例如附件），可於2022年10月31日前遞交。 |
